Insurance Information

Physical Therapy Health Services accepts but is not limited to the following insurances:
 
• Aetna
• Blue Cross Blue Shield
• GIC
• Harvard Pilgrim
• Medicare
• Medicare Railroad
• Most Private Insurances
• Motor Vehicle Accident
• Tufts
• United Health Care
• Workers Compensation
 
We recommend that you call your insurance company to verify your benefits before your first appointment. Unfortunately, we are unable to accept Mass Health, Neighborhood Health, Mass Dept of Labor or Fallon.
 
If you are covered under workers compensation or a motor vehicle accident, please let us know when you schedule your appointment. We will need your claim number, date of injury, and adjuster’s name and phone number when you schedule the evaluation so that we can verify benefits and obtain authorization for visits.

 What else do I need for my first appointment?
 
• Your physician’s prescription
• An insurance referral
• Your insurance card
• Copayments are also due each visit. PTHS accepts cash, check, and credit cards.
 
Please bring or wear shorts and/or tank top and arrive 10 minutes early to your appointment.
